All requests for facility, field, and equipment usage must be paid (7) calendar days prior to the 1st day of the event. Payments will be collected electronically. FB Sports does not accept personal checks.

Once the event has been approved, and all fees are paid, NO CHANGES may be made, unless approved by FB Sports Department. (may incur additional fees)

No event advertisements or invitations referencing “Freedom Fields”, “First Bossier”, “FB Sports”, “2810 East Texas St.”, or the like, may be used without prior approval from FB Sports Department.

Any misrepresentation in this application or deviation from the final agreed-upon activities and/or method of operation described herein may result in the immediate revocation of the permit and termination of the contract, and/or cancellation of the event. If you have any questions, please call (318) 746 -7940.

Bossier City Police Department services are required for ALL large events and/or spectator events. Officer(s) must be in place for the duration of the rental. For events with 75+ spectators, multiple officers/personnel may be required. Security fees will be added when necessary and will be the financial responsibility of the renter.

A cleaning fee will be assessed for ALL large events and/or spectated events.

Field/ Complex Rules and Restrictions

Items used on the turf surfaces are not allowed to be staked down, adhered or attached (to include tents and canopies)

Space heaters are not allowed inside the complex

Outside food and drinks are subject to inspection and approval

Coolers, backpacks and bags are subject to inspection

NO Gum on the premises

NO sunflower seeds, pistachio shells, peanut shells or other nut shells/seeds allowed inside the complex

NO Animals (no exceptions)

NO Excessive noise makers

NO Smoking or vaping of any kind

NO Tobacco Products or dipping inside the complex

NO Alcoholic beverages are to be sold, served or consumed on the premises

NO weapons allowed inside the complex (to include open or concealed carry)

All adherence to the Field and Complex rules will be assumed by and the responsibility of the renter.

Insurance Information

Do you have Certificate Of Liability Insurance? *

For the application to be approved, the applicant must submit an Approved Certificate of Liability Insurance naming First Bossier as the additional insured. The Certificate must have First Bossier as the certificate holder. The insured must match the Producing Organization or Applicant’s Name and information. The Certificate of insurance must be submitted with this permit application. The time limit on the policy must include set-up and take-down times, as well as the time of the event. The beginning of the set-up is to include deliveries prior to the event date, such as tents, flowers, decorations, etc. User’s insurance is to be written by companies licensed to do business in the State of Louisiana at the time the policies are issued and will be written by companies with A.M. Best ratings of B+VII or better otherwise acceptable First Bossier. All insurance policies must have a (30) day Notice of Cancellation Endorsement, Waiver of Subrogation, and First Bossier as the Additional Insured.
The Applicant will, at his/her own expense, provide and maintain certain insurance in full force and effect at all times during the term of the agreement. Such insurance, at a minimum, must include the following coverages and limits of liability.

Terms of Agreement

Applicants (Organizations/Applicant) will indemnify, defend, and hold harmless First Bossier, its employees, and authorized volunteers from and against all claims, damages, losses, and expenses, including attorney’s fees, arising out of the permitted activity or conduct of permittee’s operation of the event if such claim (1) is attributable to personal injury, bodily injury, disease, or death, or injury to or destruction of property, including the loss of uses thereof, from and (2) is not caused by any negligent act or omission or willful misconduct of First Bossier or its employees acting within the scope of their employment.

If any part of this application IS NOT TRUE, then the application WILL BE REJECTED and/or the contract WILL BE VOIDED. I understand that it is my responsibility to ensure compliance with the following:

a) The observance of all applicable laws and ordinances

b) Any stipulations or restrictions of the use of the fields

c) The applicant will assume any and all liabilities that may arise by the permitted activity.

d) Applicant will notify FBSports of any/all changes of the information given in this application ten (10) days

prior to the first day of the event.
